To help streamline your first appointment we offer the option of downloading and completing your Initial Client Forms before your appointment.
Please note that we must have this form signed & completed BEFORE we can begin your first session. If you are coming in-person, you can bring the forms with you to your first appointment or simply arrive 15 minutes early to fill out this paperwork in our waiting room. If your first session will be a Telehealth session you will need to fax these forms to us at 570.884.4662, email fax@rootedrt.com, or complete them online via our secure client portal.
